What people usually call “inner ear crystals” is Benign Paroxysmal Positional Vertigo (BPPV). The “crystals” are tiny calcium carbonate particles (otoconia) that have moved into the wrong part of the inner ear and mess with your balance system.
The good news: in most cases, it’s treatable with simple head-positioning maneuvers.
1) The main fix: repositioning maneuvers
These are designed to move the crystals back where they belong.
Epley Maneuver (most common)
Used especially for dizziness when turning your head or lying down.
Basic idea:
- You move your head through a series of positions while lying down
- This uses gravity to guide the crystals out of the semicircular canal
It’s usually done like this (for right ear BPPV example):
- Sit on a bed, turn head 45° to the right
- Lie back quickly with head slightly hanging off the bed
- Hold 30–60 seconds
- Turn head 90° to the left without lifting it
- Hold
- Roll onto your left side
- Slowly sit up
A clinician (ENT or physiotherapist) can do it first time to make sure it’s correct.
Semont maneuver (alternative)
Faster, more abrupt movement between lying positions. Also effective but can feel intense.
Brandt-Daroff exercises (home training)
- Less precise but helpful for recurrence
- Done multiple times daily for several days
2) What helps symptoms while it settles
- Move slowly when getting up or turning your head
- Avoid sudden head tilts
- Sleep with head slightly elevated
- Stay hydrated
- Take breaks if dizziness triggers nausea
Some doctors may prescribe short-term anti-vertigo or anti-nausea medication, but these don’t fix the cause—just the symptoms.
3) When to see a doctor (important)
Get checked urgently if you have:
- Weakness, numbness, slurred speech
- Severe headache
- Continuous vertigo that doesn’t change with position
- Hearing loss or ear pain
- First-ever severe episode (to rule out other causes)
Because not all vertigo is BPPV—conditions like vestibular neuritis, Ménière’s disease, or neurological issues can mimic it.
4) Key point
If it is BPPV, it often improves dramatically after 1–2 proper repositioning sessions. Some people feel relief within minutes to days.
